I understand tattoos carry risks including infection, allergic reaction, scarring and that results may vary.I understand a tattoo is permanent and removal is costly and may be incomplete.I have reviewed the design, placement and spelling, and approve them.I will follow the aftercare instructions provided by the studio.The studio may photograph my tattoo for its portfolio (optional).
